Your primary care physician faxes Referral Forms (1) and (2) to
Hyogo Ion Beam Medical Center

(Hours 9:00 am - 5:00 pm JST)
↓
Appointment date and time will be faxed to the primary care physician
↓
First examination at Ion Beam Medical Center
Assessment of eligibility
Explanation of radiation therapy
↓
Therapy begins as outpatient or inpatient
